Colorado Health Network Food Bank Eligibility Requirements
There is a large parking lot in front of the building and there is a bus stop in front as well. Our food bank is located on the lower level of the building. Eligible clients can check in with the front desk and present their client cards to get access.
Eligibility Requirements:
- Participants must be at or below 200% of Federal Poverty Line (FPL) and enrollment into CHN services is required
- Generally, clients in Ryan White programs update annually during their birth month, and all other clients get updated in Link2Feed annually or as needed
- Clients living with HIV/AIDS must be enrolled in Ryan White Case Management services to access our full shop (both TEFAP and Ryan White funded items)
- In order to get enrolled, you can reach out to Chase Conlin by calling 303-962-5314 or by emailing Chase at Chase.Conlin@coloradohealthnetwork.org with their proof of diagnosis (HIV+ lab results or physicians letter). Additionally, you can reach out to their front desk by calling 303-837-0166.
- If participants are already enrolled with another AIDS Service Organization (ASO), their case manager can email/fax Chase Conlin (Chase.Conlin@coloradohealthnetwork.org) a food bank collaboration request.
Additionally, participants who access our prevention services through CHN’s AccessPoint program can only access TEFAP items. You can obtain a food bank card from AccessPoint by engaging even just once with that program where you can either obtain harm reduction supplies or utilize free STD/STI testing. The regular hours for the food bank are below. They may have an altered schedule due to holidays or special events.
- Tuesday: 11:00am – 4:00pm
- Wednesday: 1:00pm – 5:00pm
- Thursday: 11:00am – 4:00pm
