Oak Street Health – Kate Morris

Provides personalized primary care for adults on Medicare, with a focus on older and low-income adults.
Offers longer appointments, same-day/next-day appointments, and consistent provider/MA relationships.
Provides support related to mental health and housing stability.
Adams County currently has 2 doctors serving approximately 500 patients, with another doctor potentially being added.
Current waitlist in Adams County is approximately 2 weeks.
Transportation remains a significant barrier in Adams County, particularly for patients living outside the 10-mile service area.
Provides transportation assistance through Medicare benefits and Lyft when available.
Patients may receive up to 20 rides to medical appointments.
Wheelchair-accessible transportation (WAV) after hours remains a challenge.

Thornton Active Adult Center – Marisa Kouba

Serves adults 55+, with fitness benefits available to many adults 62+.
Offers fitness classes, wellness services, art classes, meals, and social activities.
Hot Meal Program provides approximately 120 meals daily at $4.25 per meal.
Offers weekly trips and hikes using 2 buses, each serving up to 15 people.
Transportation remains a high-need area with limited availability.
Three city buses provide transportation, but home pickup is generally limited to participants in the food program.
Via Mobility regularly provides services at the center.
The center is focused on helping older adults remain active, safe, supported, and connected to their community.
Transportation alternatives such as e-bikes and pedal-assist tricycles were discussed as important options for older adults.
Notable success: Adults 62+ participated in hiking a 15er this past year.

Adams County Human Services:
- Continuing to utilize A-Lift through Via Mobility.
- Some areas have free rides with no waitlist.
- Other areas continue to experience ride denials due to capacity.
- Rides can be scheduled at any time.
- UCHealth Fitzsimons rides are currently not in effect.
